From 1982 to 1984 eleven patients with thrombosed PTFE femoro popliteal bypass grafts were treated by thrombectomy with a high incidence of rethrom bosis (1.4 occlusion per patient) resulting in four amputations. Since 1984 a selective surgical approach was instituted depending upon angiographic evi dence of inflow problems, intimal hyperplasia or distal progression of athero sclerosis. No amputation and only one episode of rethrombosis occurred in a group of nine patients. We concluded that the natural history of thrombectomy for failed F-P bypasses is rethrombosis and that a selective approach should be individualized for each patient.
