Active Expectant Management of Preterm Premature Rupture of Fetal Membranes (PROM) with Cervical Cerclage

Previous experience has shown that an ‘active expectant management’ protocol consisting of prophylactic antibacterial agents, tocolysis and intensive fetal monitoring is superior to passive expectant management in patients with premature rupture of fetal membranes (PROM). Active expectant management was utilised in 3 patients with PROM and a cervical cerclage left in place. The latent phases for the 3 patients were 4.6, 14.3 and 30.25 days, respectively. No relationship was noted between C-reactive protein levels or WBC and clinical outcome. Pseudomembranous enterocolitis developed in 1 patient and was successfully treated without stopping prophylactic antibacterial therapy or delivering the fetus. A low rate of infection-related morbidity was observed.
